APPLY TO FBC ACADEMY SCHOOLS

Registration Fee: $200 (Non-Refundable) - This fee is required to begin the Admissions process

    An application remains incomplete without the submission of the most recent immunization record.  FBC Academy follows the Texas Immunizations requirements for Preschool -12th grade in accordance with the Texas Department of Human Health Services. 


    Before students start their first day of preschool they must have completed the following Preschool Immuniztion Requirements:

    •    Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) — 4 doses 

    •    Polio (OPV or IPV) — 3 doses 

    •    Hepatitis B — 3 doses 

    •    Measles, Mumps, and Rubella (MMR) — 1 dose

    •    Varicella (Chickenpox) — 1 dose

    •    Hepatitis A — 1 dose 


    Before students start their first day of Elementary school they must have completed the following Elementary Immunization Requirements:

    •    Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) — 5 doses 

    •    Polio (OPV or IPV) — 4 doses 

    •    Hepatitis B — 3 doses 

    •    Measles, Mumps, and Rubella (MMR) — 2 doses 

    •    Varicella (Chickenpox) — 2 doses 


    Students entering Junior High must have completed all Elementary immunizations.  Before students begin their first day of 7th grade they must complete the 7th Grade Immunization Requirements:

    • Meningococcal —1 dose  


    Students entering High school must have completed all Elementary & 7th grade immunizations before the first day of school.

  • Immunization Exemption

    Texas law allows (a) physicians to write medical exemption statements which clearly state a medical reason exists that the person cannot receive specific

    vaccines, and (b) parents/guardians to choose an exemption from immunization requirements for reasons of conscience, including a religious belief. The

    law does not allow parents/guardians to elect an exemption simply because of inconvenience (for example, a record is lost or incomplete and it is too

    much trouble to go to a physician or clinic to correct the problem).


    Instructions for requesting the official exemption affidavit that must be signed by parents/guardians choosing the exemption for reasons of conscience,

    including a religious belief, can be found at dshs.texas.gov/immunization/school/exemptions . The original Exemption Affidavit must be completed and submitted to the school.


    For children claiming medical exemptions, a written statement by the physician must be submitted to the school. Unless it is written in the statement that a lifelong condition exists, the exemption statement is valid for only one year from the date signed by the physician.

  • Transcripts

    Submit a transcript of the last two years. They must include grades for the current school year and the complete previous year. For current school year grades, the Administration will accept parent report cards, quarter grades, or progress reports. However, the Official Transcript must be submitted prior to beginning their first term at FBC Academy. For Kindergarten applicants, please submit a pre-school progress report (if applicable).


    If required by your current school, we can provide a Transcript and Standardized Testing Request Form.

  • Student Support Documentation (if applicable)

    Due to staffing ratios, FBC Academy is limited in its ability to provide individualized instruction to students that require accommodation and/or modification of curriculum. If a student can be successful in a regular education classroom with the teacher implementing limited interventions, the student may be considered for admission on a case-by-case basis. Parents must disclose any learning support or services their child requires at the time of application for admission, including submission of the child’s most recent documentation such as: 

    •    IEP, 504, Accommodation Plan, Education Plan, Service Plan or similar documents from you student's current school.

    •    Your student’s official diagnosis documentation from a licensed professional.


    Should a parent fail to disclose his or her child’s special needs or the need for services, the Administration reserves the right to remove the student from the program and tuition will not be refunded.

STEP THREE: Follow-up Interviews/Conference


After all of your application documents have been received, FBC Academy Administration may contact you to schedule a family interview if needed.
